CONGRATULATIONS! If you have been asked to participate in this survey it means that you are working with a team of educators who are committed to creating an inclusive working and learning environment. It also means that they want to ensure that both team members such as yourself, as well as students feel appreciated for your individuality and valued for your unique contributions.

Your participation in this survey is a critical first step in helping to create a culture of diversity and understanding within your school. It is an opportunity for you to share your observations transparently. The information you provide will help identify organizational competency gaps that will be used to develop an action plan for continuous improvement. 

Your individual responses to the questions contained in this survey will remain anonymous. Your feedback will be used to establish a baseline performance grade of your school. As a result it is important that your answers accurately reflect your true beliefs and experiences as it relates to diversity, equity, and inclusion. Demographic and team data is gathered solely as a measurement tool to identify common trends or concerns among various subgroups. 

There are no right or wrong answers. Choose the answer that most closely represents your personal opinions and experiences. You will also be given an opportunity at the end of several questions to provide additional details related to your answers. This added information is important to be able to draw a complete picture of the experience. At no time will any identifying information be shared with anyone within your school or district.
