Your participation in this survey is a critical first step in helping to create a culture of diversity and understanding within your organization. It is an opportunity for you to share your observations transparently. The information you provide will help identify organizational competency gaps that will be used to develop an action plan for continuous improvement. 

Your individual responses to the questions contained in this survey will remain anonymous. Your feedback will be used to establish a baseline performance rating of your organization. As a result it is important that your answers accurately reflect your true beliefs and experiences as it relates to diversity, equity, and inclusion. Demographic and team data is gathered solely as a measurement tool to identify common trends or concerns among various subgroups.
